Treating Polycystic Ovary Syndrome with Ayurveda
We will see how Ayurveda brings added depth to the understanding of this poorly understood condition and can offer treatment options that are more than merely symptomatic.
PCOS is a syndrome characterized by multiple small cysts on the ovaries, menstrual irregularities and features of excess androgen production such as hirsutism (excess facial or body hair) and acne.
According to Astanga Hrdayam, twenty disorders of the female reproductive tract exist, caused by poor flow, defective ovum and ovary, and past karma. This results in an inability to conceive and other health concerns such as tumors, polycystic ovarian syndrome, hemorrhoids, menorrhagia, endometriosis, etc. Ayurveda observes the disease looking for indications of the dosha responsible for the disorder.
Doshas, Subdoshas, and Srotas Involved In PCOS Pathology :
Any imbalance or diseased condition is seen as interplay and imbalance between the doshas. Each of the three doshas Vata, Pitta, and Kapha have a specific role in the female reproductive cycle. Vata dosha rules the reproductive system, and the organs are located in artava dhatu, these are the tissues of the reproductive organs.
Artava provides nourishment for the ovum and allows movement of the egg from the ovum to the uterus. Vata, “is responsible for movement of the follicle during the ovarian cycle, the rupture of the ovary wall releasing the matured ovum, the movement of the fimbriae – the finger like projections that guide the ovum into the fallopian tubes and the movement of the ovum towards the uterus.” Apana vayu, one of the five subdoshas of vata, allows for the downward movement of menstruation fluid and delivery of a child during labor. Pitta is responsible for transformation, and influences hormonal balances. Kapha dosha is responsible for providing nourishment and growth of the tissues such as the growth of follicles, uterus, and the development of an egg and sperm into a fetus.
PCOS is a sannipatika or tridoshic condition. The disease begins as a vata imbalance, which pushes kapha and pitta out of balance in the shukra vaha srota, or reproductive channel. When vata becomes vitiated in the shukra vaha srota it manifests as an irregular menstrual cycle. Pitta vitiation in leads to acne, hirsutism, and hormonal imbalances including excess circulating testosterone in the body. Kapha vitiation leads to symptoms associated with cysts, and weight gain.
PCOS affects all three of the doshas, so treatment of all three doshas is cornerstone in being able to treat and manage this disease.
In Ayurveda, the state of balance of doshas is main component responsible for health and any disruption to this state will lead to disease. 
All three doshas (Vata, Pitta, Kapha) play important and distinctive roles in the processes behind the female reproductions which includes the ovarian cycle and the menstrual cycle. PCOS is due to the Kapha blocking Vata and Pitta, hence movement is obstructed and the transformation process is suppressed.
Ayurveda suggests that this is vata type disorder (Apan vayu) though the involvement of other dosha can be there but in some measure because the gynaecological disorder are mainly supposed to be due to vitiation of vata.Pcos Is A Disorder Involving Pitta, Kapha, Medas,Ambhuvahasrotas & Artava Dhatu. The causes of PCOS as per ayurveda can be taken as eating excessive sweet and kaphagenic foods, mandagni because of this is kapha getting aggrevated in PCOS, we find kapha disorder. As well as pitta and vata dosha disorder. Because of all three doshas play important & distinctive role in the production, development, maturation & release of ovum & therefore the ovarian cycle & menstrual cycle is under control of three doshas.
Kapha having the first affected the digestive fire, jathara agni starts to affect the metabolic aspect of the seven tissues of dhatu agni. Each dhatu agni is responsible for the nourishment and formation of that particular tissue that resides in. In the case of PCOS the dhatus that are affected are rasa dhatu, lymph and plasma, meda dhatu, the adipose tissue and artava dhatu, the female reproductive system.
As the heavy cold sticky qualities of Kapha suppress the digestive fire, food that is ingested is not properly digested forming ama. Due to the sticky heavy qualities of Kapha the cell membrane of the tissues is coated suffocating the agni affecting the cellular intelligence causing insulin receptors on the cell to not recognize chemical structures that normally engage them. Artava dhatu affected by the heavy sticky qualities of kapha and ama creates srota dushti in artavavaha srota. Apana vayu in artavavaha sorta (The channel that supplies, nourishes and enables the functional action of carrying the ovum to the uterus is called artavavaha srota) becomes stagnant – sanga, due to excessive kapha and ama accumulation blocking the channel impeding the flow of vata in the ovarian cycle. Because vata is blocked, pitta is blocked as well as pitta in order to act as the intelligence behind transformation needs the movement of vata in order for its energy to have potential.
Although there may be pitta features such as acne and hair loss or vata features like menstrual irregularity, it is kapha that lies at the root of this syndrome. Although PCOS may affect any prakruti, as a kapha condition it is typically most severe in kapha prakruti. A majority of all women with PCOS have been of kapha prakruti.
The first step in creating balance in someone with PCOS is addressing the Ama, as we know PCOS is an imbalance related to the Ama created from Kapha. 
The powder of herbal medicines are also quite beneficial in curbing the three aggravated doshas & brings balance & strength to the menstrual system & it helps to regulate artava dhatu. 
PANCHAKARMA..
Pancha Karma consists of five therapies which helps to clear ama and balance agni. Panchakarma is the detoxification and rejuvenation therapy has proved quite successful in managing PCOD symptoms.
Panchkarama protocol helps to high degrees to clear many pathologies which cause a direct inhibition to fertility. Basti treatment proves very beneficial. Since Basti is targeted at regulating the Apana Vayu it facilitates the timely release of ovum and also the good production of sperms. Uttar Basti, is helping to strengthen the uterus and help implantation. Purificatory process of Vamana and Virechana also help in patients with a high imbalance of Dosha. Nasya is an important Karma to promote fertility. Nasya with my herbal ghee is very benficial in women for timely release of an egg, as Nasya is claimed to act directly on the hormonal apparatus. Abhyanga body treatment with Vata calming oils and Swedana with Vata herbs will also be beneficial. 
The five treatments include:
-Vamana, which is induced vomiting. It is recommended in the treatment of weight loss as it aids in the removal of excess kapha dosha.
-Virechana, or purgation therapy alleviates pitta by removing heat in the small intestine and liver, which aids in balancing hormonal levels in the body.
- Basti, or enema, is another therapy used in Pancha Karma treatment. Both uttara and rectal bastis are effective in treating disease in the vagina and uterus. “In diseases of the urinary bladder and of vagina and uterus in women, uttarabasti should be administered, to those who have purified by two or three decoction enemas, per rectum.
Bastis treat amenorrhea and hypomennorhea by clearing obstructions and restoring flow to apana vayu and assist with the elimination of “accumulated impurity stationed all over the body. Basti regulates the functioning of the hypothalamus which normalizes menstruation.
-Oleation and fomentation help to cleanse purify the body.
Oleating the body internally with ghee and externally with oil the body softens ama and toxins in the body. Fomentation liquefies the toxins and removes it from their sites in the body leading to clearer channels and improved health. Charaka Samhita states that diseases of vata origin in the vagina benefit from oleating, fomentation and enema.
A woman with PCOS may present complaining of hirsutism, acne, obesity or menstrual and fertility issues.
